We had a great FCSN presentation on Basic Rights: Transition on March 11th! The IEP for the year during which the student turns 14 should contain transition information. This is the student’s vision for their future along with planned assessments (career aptitude, skills inventory, and academic) and how the student will attain the skills – academic and non-academic – needed to manage a positive transition into college and/or the workforce. Parents/guardians are encouraged to work with the child and the school to come up with a practical plan of where/how the student can work on these identified areas. We’ve attached the TPF form, which is an IEP planning form used for transitions.
